and efficient interventions that help individuals
maximize their strengths and lead rewarding

Who
Benefits?
People returning to
the community after inpatient treatment for
a mental illness; people with psychiatric disabilities
that have been incarcerated or are involved
with the legal system; and the homeless.
What
do we offer?
Intensive,
community-based services. We assist individuals
with all aspects of living in the community
to help them gain stable housing, improved quality
of life, fewer crises and hospitalizations.
How
does it work?
Community Support staff are mobile and work
in close partnership with individuals to develop
service plans based on individual goals.
Case
Management
Our case management
staff works with the individual and their family
to identify and prioritize needs. Together,
they develop a service plan that establishes
short and long-term goals and objectives for
achieving them.s
F.A.C.T.
(Florida Assertive Community Treatment) Teams recognized program for delivering “services without walls.” We recognize the therapeutic role of the home environment for providing community-based psychiatric rehabilitation, treatment and intensive case management services to persons who have not responded well to traditional mental health services. Services are provided by multi-disciplinary teams and are available 24/7.
